What Is the TOVA Test for ADHD?

The TOVA — Test of Variables of Attention — is an FDA-cleared, computerized assessment used to measure attention and impulse control in adults and children being evaluated for ADHD. It takes about 22 minutes, requires no reading or language skills, and produces objective data on inattention, impulsivity, response time, and response time variability. The TOVA is one tool inside a broader ADHD evaluation — not a stand-alone diagnostic test.

How the TOVA Actually Works

The TOVA is a continuous performance test (CPT) — a category of computerized attention tests used in psychiatric and neuropsychological evaluation. During the test, simple visual stimuli (a square with a smaller square either on top or on bottom) flash on the screen for about 100 milliseconds. That sounds easy. It isn’t, particularly for someone with attention regulation problems. Over 22 minutes, the test measures four core variables:

• Errors of omission (missing a target you should have responded to) — measures inattention
• Errors of commission (responding to a non-target you should have ignored) — measures impulsivity
• Response time — measures processing speed
• Response time variability — measures consistency, often the most sensitive indicator of ADHD

The test has two parts. The first half presents targets infrequently, simulating a low-stimulation environment where attention is the challenge. The second half presents targets frequently, simulating a high-demand environment where impulse control is the challenge. Together, they probe both sides of attention regulation.

Results are compared against age-matched and gender-matched normative data. The TOVA software produces standardized scores and an overall “ADHD Score” derived from a discriminant function analysis.

What the TOVA Adds to an ADHD Evaluation

ADHD diagnosis is fundamentally clinical. It rests on a structured interview that maps symptoms onto DSM-5-TR criteria, history-taking (including symptoms present before age 12), validated rating scales (like the ASRS or Conners), and observation. No single test diagnoses ADHD by itself.

What the TOVA contributes is objective measurement of attention and impulsivity that doesn’t depend on self-report. That matters in several specific situations:

Self-report uncertainty. Some adults have a hard time describing their attention problems, or wonder whether what they’re experiencing is “really” ADHD or just being overworked, anxious, or under-slept. Objective data helps clarify. We cover how long an ADHD diagnosis takes in a separate article.

Differentiating from anxiety or depression. Both anxiety and depression can mimic ADHD attention symptoms. The TOVA pattern often differs across these conditions — and that pattern, combined with clinical interview, sharpens the diagnostic picture.

Establishing a treatment baseline. When medication is started, repeating the TOVA on and off medication produces a clean before-after comparison of how attention has changed objectively. This is one of its most useful clinical applications.

What the TOVA does not do: confirm or rule out ADHD on its own. A normal TOVA does not mean a person doesn’t have ADHD. An abnormal TOVA does not confirm they do. It is one input in a multi-source clinical decision.

When the TOVA Is Ordered

Not every ADHD evaluation includes a TOVA. It’s most useful in cases where:

• Diagnostic uncertainty exists after clinical interview and rating scales
• Anxiety or depression complicates the symptom picture
• The patient wants objective baseline data before starting medication
• Insurance or employment requires objective documentation
• A baseline-and-follow-up comparison would help guide treatment decisions

Some evaluations don’t need it — when the clinical picture and history are clear, the test may not change the plan. The decision to order TOVA is made during the evaluation based on the individual case.

Most adult ADHD evaluations combine a clinical interview with one or more validated rating scales. The TOVA is added when objective attention measurement would change or sharpen the diagnostic decision.

What the Day of TOVA Testing Looks Like

The test runs on dedicated software with a hand-held response button. The room should be quiet, free of distractions, and minimally lit. A few practical notes:

• The test cannot be taken if you’ve had caffeine in the prior 3-4 hours, since caffeine measurably affects attention metrics
• If you’re already on stimulant medication, your provider will tell you whether to take it that day — usually based on whether the test is establishing a baseline or measuring response
• Sleep deprivation, illness, or significant anxiety affect results, so the test is rescheduled if any of those are active
• Children and adults take essentially the same test, with age-matched scoring

After the test, the software generates a structured report. The clinician interprets the report alongside the rest of the evaluation rather than handing the patient the raw numbers.

Is the TOVA test accurate for diagnosing ADHD?
The TOVA is accurate as one input into a clinical evaluation, not as a stand-alone diagnostic. Sensitivity and specificity are reasonable when combined with clinical interview and rating scales. It’s not designed to make a diagnosis by itself.

How long does the TOVA test take?

The TOVA itself requires the proper hardware and a controlled environment, so it’s typically done in-office.
