From 16 years at Bay Pines VA to founding Trust in 2023.
Trust Psychiatry & Wellness was built on a deliberately different premise than most adult psychiatric care in West Palm Beach — one clinician, one chart, one cadence, billed in-network instead of cash-only.
Josie Desmarais, PMHNP-BC, spent 16 years (2007–2023) at Bay Pines VA Healthcare System — first in inpatient psychiatry, then in outpatient mental health, and finally in MAT for veterans living with co-occurring opioid use disorder and PTSD. The Bay Pines arc shaped how Trust delivers care: same prescriber every visit, written treatment plan, evidence-based regimens drawn from APA Clinical Practice Guidelines, and honest timeframes about when medication actually starts working.
The gap that prompted Trust was specific to West Palm Beach adult psychiatric care. Most prescribers in the area run cash-only or out-of-network. Medication and therapy get split between a prescriber the patient sees for 15 minutes every three months and a separate counselor who never sees the chart. Continuity collapses. Diagnoses drift. Patients end up retelling the same history every visit and starting over every six months.

Prescribing scope. Full FL APRN prescribing authority across Schedule II–V, monitored through Florida E-FORCSE Prescription Drug Monitoring Program. Buprenorphine (Suboxone) prescribing capacity for opioid use disorder under DEA Schedule III registration. Schedule II stimulants for adult ADHD case-by-case after clinical workup — not first-visit prescribing, and only after history, rating-scale data, and prior-trial documentation align. Schedule IV benzodiazepines and sleep medications case-by-case under E-FORCSE monitoring — generally not first-line, with non-controlled alternatives preferred where clinically appropriate.
Clinical posture. Same-day available — no children, no adolescents, no family or couples work. Medication-management-led, paired with supportive psychotherapy in the same relationship. Measurement-based care across visits. Honest about scope — Trust does not provide inpatient admission, IOP, PHP, or residential treatment; when a patient’s picture exceeds outpatient scope, Trust coordinates referrals to the right level of care rather than stretching beyond ours. The goal of every visit is one of three outcomes: change in plan, change in dose, or steady-state continuation backed by data.

Transparent billing. The initial evaluation is the first clinical visit and is billed to insurance after benefits are verified. Trust does not offer free consultations — the initial evaluation generates a chart, a diagnosis, and a plan, so it is billed like any other clinical visit. Patient responsibility is disclosed before each visit. No surprise balance billing.
