HIPAA intake forms via Kareo portal
After booking, the Kareo patient portal emails secure forms — history, current medications, pharmacy of choice, photo ID and insurance card upload. Encrypted at rest and in transit. Never share clinical detail by email or text.
30–60 minute psychiatric evaluation
In-office at 4500 Belvedere Rd or by secure telepsychiatry video. Josie reviews history, sets a care plan, and (when clinically appropriate) e-prescribes to your local pharmacy. Follow-up cadence set the same day.

Do I need a referral to book?
No referral is required. Adult patients book directly through the Kareo scheduler above or by calling (561) 849-4449. Some Florida HMO plans request a PCP referral on file for in-network billing — we verify your plan’s requirement when we check benefits.
How fast can I get in?
Most new evaluations are scheduled within 7–10 business days. Same-day slots open when availability permits, and evening plus weekend hours are offered. If the Kareo calendar looks full, call us — a few slots for urgent intakes are held offline.
In-person at the office or telepsychiatry?
You choose. In-office visits run from 4500 Belvedere Rd, Suite D. Telepsychiatry is delivered by secure HIPAA-compliant video to any adult living in Florida — all 67 counties. Some clinical situations are easier in person on the first visit (especially MAT induction); the intake call clarifies the right format.
Why Kareo Tebra and not a generic contact form?
Generic contact forms are not HIPAA-compliant for clinical detail. Kareo Tebra is the certified EHR Josie uses for charting, e-prescribing, and billing — one platform from first click to first refill, with a signed Business Associate Agreement, encrypted patient portal, and audit logs.

Which Florida plans are in-network?
Trust is in-network with Aetna, Cigna, UnitedHealthcare, Optum, Florida Medicaid, AvMed, Evernorth, TRICARE, M-Care, Cuare, HP, and Paramount. Benefits are verified before your first appointment so there are no out-of-pocket surprises at intake.
I have BCBS Florida or Humana — can I still see Josie?
Yes. BCBS Florida and Humana are credentialing in progress directly with Trust. In the meantime, both plans are covered in-network via the Grow Therapy bridge — same provider, same clinical model, full benefits applied at intake. Once Trust’s direct contract closes, we switch you to direct billing.
What does the visit cost if I’m uninsured?
Self-pay rates are published on the Insurance and Fees page. Nothing is collected until benefits are verified or the self-pay rate is confirmed with you in writing.

Is the Kareo scheduler HIPAA-compliant?
Yes. Kareo Tebra operates under a signed Business Associate Agreement with Trust Psychiatry & Wellness. Booking, intake forms, ID and insurance card uploads, telepsychiatry video, and patient messaging all run through the encrypted patient portal — never plain email or SMS.
What about MAT or SUD records?
Substance use disorder records receive a higher protection standard under 42 CFR Part 2, a federal confidentiality rule that exceeds standard HIPAA. Written patient consent is required before any disclosure, with limited federal exceptions — nothing is released to employers, family, or third parties without your signature.
